Premature Rupture of Membranes and Its Effect on Neonatal Sepsis and Morbidity and Motality.

Abstract
A clinical study was carried out to evaluate the effect of PROM on neonatal sepsis and neonatal motality and morbidity on 1,539 newborns with PROM and 17, 718 newborns of without PROM at Taeku catholic University Hospital during the three year
period
from January 1988 to December 1990
The overall incidence of PROM was 11.6% with little yearly variations.
12. 1% of all babies with PROM was born prematurely, significantly higher than the incidence of control group (2.9%) and the incidence of birth weight below 2500gm in PROM group(f16.9%) was significantly higher than in control group(5.1%).
Mobidity and motality incidence in the PROM group were 6.1% due to sepsis, 5.5% due to asphyxia, 0.2% due to RDS and 1.6% due to neonatal death and the incidence of sepsis, asphyxia and neonatal death in the PROM group was significantly higher
than
in
the control group. The incidence of Neonatal sepsis, asphyxia and neonatal death was increased as the duration of the latent period was increased and the critical time was PROM more than 24hrs.
The risk of neonatal sepsis was increased in newborns with asphyxia, gestional age below 37 weeks, meconium stained, PMN/HPF>5 in gastric aspirates and male sex. In laboratory findings, CRP¡Ã1+, ITN¡Ã0.2, IMN¡Ã0.3, toxic granule and vacuolization
of
WBC, total WBC count<5,000/§§were very useful in early diagnosis of neonatal sepsis. In gastric aspirate culture, E. coli, Enterobacter sp., Serratia sp. Were common pathogens and in blood culture, S. epidermidis, Achromdbacter xylosoxidans, K.
Pnemoniae, Enterobacter sp. were common pathogens. In urine culture, E. coli, K. Pneu-monia, enterobacter sp. were common organisms and in CSF culture, E. coli and S. aureus were cultured.
